What to expect:

The partnership between you and your physical therapist is evident from your first visit to Optimal Results Physical Therapy in Portland, Oregon.

We will take sufficient time to conduct a thorough evaluation of your injury or condition and to discuss with you treatment options and clearly outline our recommendations.

You can expect to receive your first therapeutic treatment on your first visit, so please wear loose-fitting, comfortable clothing.

If you have notes or x-rays from your doctor, please bring them as well as a list of any prescribed or over-the-counter medication you are taking regularly.

If you normally wear corrective lenses or hearing aids, please ensure they are in place. The communication between you and your physical therapist is vitally important for your treatment to be most effective.

Please bring your insurance card as well. To save time on your first visit, you may also wish to download the patient forms below, fill them out and bring them with you.
